At Harvest Family Health, pricing is simple and transparent. You only pay for the services you receive – no memberships, no hidden fees.
HSA/FSA funds may be used for visits. We do not accept or bill insurance; however, you may still use your insurance to cover services with other providers and facilities for labs, imaging, prescriptions, and specialist care.
Late Payment Fees: Payment is due at the time of service. For your convenience, we accept ACH and credit cards only. Appointments cancelled more than 24 hours prior to the scheduled time will not be charged a late fee. Appointments canceled within 24 hours of the scheduled time will incur a $25 late cancellation fee. Failure to appear for appointments will result in a $40 no-show fee.
Note: Clinic appointments are made in 30- or 60-minute increments. Telehealth appointments are usually shorter in nature, depending on the type of visit.
This visit is for school-age childreen (5-12) and teens. This is typically a preventative visit that includes a review of your child's medical history, a physical exam, and management of any ongoing concerns (e.g., acne, asthma). Children must be accompanied by a parent or guardian.
This visit is for adults and teens who have graduated or no longer attend high school. This is typically a preventative visit that includes a comprehensive review of your medical history and includes a physical exam, management of chronic conditions (e.g. high blood pressure, depression) and creation of a health plan for the year.
This visit will typically involve a single concern and does not require a follow-up visit, lab draw, or imaging referrals.
This visit is for a more complex concern that may require labs, imaging, or additional follow-up services or referral to specialists.
For urgent concerns, illness, and minor injuries. Typically focused on a single issue. Children and teens must be accompanied by a parent or guardian.
This visit is limited to completing the physical exam focused on school/sport requirements and paperwork required by the school district or sports team. If a sports physical is needed during a scheduled Preventative and Annual exam, the sports clearance is included in that visit.
This visit includes a mission or religious service physical that focuses on the requirements of the requesting organization. It is more targeted than a Preventative & Annual Visit.
This is for seniors needing a physical prior to missionary or religious service and completing the required paperwork. This visit may also be required for young adults who have a more complex past medical history.
Our hormone and weight management programs include a comprehensive evaluation focused on hormones and/or weight management, review of goals and symptoms, lab orders and interpretation, personalized treatment planning, guidance on medication or supplements when appropriate, and ongoing coaching and support.
This visit is the initial visit where a more comprehensive health and symptom history is collected and also includes additional education regarding reproductive aging and menopause (women) and hormone changes as you age (men). Recommended or optional lab tests will be reviewed for joint decision-making on how to proceed. An individualized plan and recommended follow-up will be established as part of this visit.
This visit will focus on evaluating your progress, adjusting your goals, plan, and/or medications, if needed. Follow-up visits occur after initiation or changes to medications. Your initial follow-up will occur at 6-8 weeks and then transitions to a follow-up visit every 3-6 months in the first year, then annually. This follow-up may be included in your more comprehensive Preventative & Annual Visit.
Home being visited is within 10 miles of clinic location. Appointment date/time of visit made in consultation with provider. This fee is added to the visit fee.
Home being visited is within 11-20 miles of clinic location. Appointment date/time of visit made in consultation with provider. This fee is added to the visit fee.
Clinic-based Labs: Testing completed during your in-person visit (e.g., urinalysis, strep) will have the cost of the test added to your visit fee. Costs range from $10 to $40, depending on the test(s) performed.
Referrals to outside labs, imaging, and diagnostic services are billed directly by the location where you received the services. HSA/FSA or insurance may be used for most services. You will receive a separate bill.
