Overview of Pricing

At Harvest Family Health, we make it easy to understand the cost of your care. You can choose to use insurance or simple, upfront cash-based pricing.

You are responsible to know if we are a part of your insurance network. If paying cash, or out of network, you may still use your insurance to cover services with other providers and facilities for labs, imaging, prescriptions, and specialist care. HSA/FSA funds may be used for visits. 

Note: Clinic appointments are made in 30- or 60-minute increments. Telehealth appointments are usually shorter in nature, depending on the type of visit.

Using Insurance

Harvest Family Health works with select insurance plans and continues to expand participation.

    • Coverage, copays, and deductibles are determined by your individual plan.
    • Claims are submitted on your behalf when we are in-network.
    • Your out-of-pocket costs will vary based on your benefits.

**If using insurance, visit fees are billed using standardized medical coding and contracted rate structures determined by your plan.

Hormones & Weight Programs

Our hormone and weight management programs include a comprehensive evaluation focused on hormones and/or weight management, review of goals and symptoms, lab orders and interpretation, personalized treatment planning, guidance on medication or supplements when appropriate, and ongoing coaching and support.

Home Visits

Testing

Clinic-based Labs: Testing completed during your in-person visit (e.g., urinalysis, strep) will have the cost of the test added to your visit fee. Costs range from $10 to $40, depending on the test(s) performed.

Referrals to outside labs, imaging, and diagnostic services are billed directly by the location where you received the services. HSA/FSA or insurance may be used for most services. You will receive a separate bill.

Late Fees

Late Payment Fees:  Payment is due at the time of service.  For your convenience, we accept ACH and credit cards only. Appointments cancelled more than 24 hours prior to the scheduled time will not be charged a late fee. Appointments canceled within 24 hours of the scheduled time will incur a $25 late cancellation fee. Failure to appear for appointments will result in a $40 no-show fee.
